APPEALS, COMPLAINTS & FEEDBACK ON PROJECTS & POLICIES

Have your complaints and appeals Scheduled and actioned directly by the appropriate office, get feedback, and follow up till done.

01. Before you start your report

Please note that a complaint is a submission to the ministry for services that are not in accordance with service standards, or the waiver of obligations and/or violations of prohibitions .

Please Ensure that:

• your report is relevant to government performance.

• you write your complaint in English (the pidgin and foreign languages version coming)

• your complaint does not use hate or insulting language.

You will receive a message responding to a previous complaint , If a similar report has been submitted and still in process of being resolved.

02. mandatory Fields

You must answer these mandatory / Required Fields

Report Title: is the conclusion of a problem, the essence of a report that is submitted

Report category: the category that corresponds to the reported report

Contents of the report: Telling the chronology of events that you want to complain about.

Date of occurrence: The date when you received unsatisfactory service

Occurrence Location: The location where you observed the issue or received unsatisfactory service (the more specific you input, the better).

03. Optional Fields

The following fields are optional

Anonymous: keeps your report from appearing on the public page.

Secret: keeps your report from appearing on public pages. Make sure you tick anonymous and confidential for reports that are sensitive and contain any personal data (including names, etc.) or the reported would be voided.

Attachments: You can support your reports in the form of pictures, documents, and videos with a maximum upload of 2 MB. You can upload more than one document.

Complete your Appeals & Complaints form clearly - following instructions in "How to submit a report correctly"

Verification Process

Within 3 days, your report will be verified and forwarded to the appropriate desks.

Follow up Process

The agency replies to your report

Give Feedback

You can respond to the reply the agency provides within 10 days of receipt

Your report will receive follow up till its resolved

You can just send a report as a guest, but You need to sign up to get feedback and interact with the process
